Location: West Kent
Salary: £24,323 - £29,951 per annum (DOE)
Role Overview:
Our client is a progressive charity dedicated to supporting disabled and disadvantaged individuals through training and employment. They manage community mental health and wellbeing services (Live Well Kent and Medway) focused on recovery and staying well. Their mission is to transform lives by providing person-centred support that improves wellbeing and community connectedness.
Key Responsibilities:
- Wellbeing Plans: Develop, implement, and review wellbeing action plans with clients, including 1-2-1 and group sessions.
- Needs Assessment: Assess client needs, identify outcomes, and determine support required to overcome barriers.
- Risk Management: Conduct and update risk assessments related to clients, activities, and environments.
- Service Referrals: Refer clients to services within funded and unfunded networks to support wellbeing goals.
- Community Signposting: Direct clients to existing community resources to support their wellbeing goals.
- Prevention Focus: Promote prevention of poor wellbeing through Making Every Contact Count (MECC).
- Case Conferences: Participate in case conferences with partner organizations and support services.
- Post-Programme Tracking: Monitor clients post-programme to ensure they maintain their wellbeing.
- Volunteer Supervision: Guide volunteers to provide additional support to clients.
- Client Journey Management: Maintain accurate administrative records and manage all aspects of the client journey.
- KPIs and Quality Service: Achieve service delivery KPIs and provide a quality service meeting 6 Ways to Wellbeing standards.
- Feedback and Involvement: Encourage client feedback and user involvement through a co-production framework.
- Stakeholder Relationships: Build relationships with local councils, housing teams, and health & social care teams.
- Local Provision Knowledge: Maintain knowledge of local services and opportunities related to wellbeing.

- SVQ/NVQ level 3 (E)
- Training in health and social care (D)
- Mental health and wellbeing interventions (E)
- Working with health/social care services (E)
- Supporting clients with complex barriers (E)
